چکیده

A early reports of erythropoiesis-stimulating agents (ESAs) having success correcting anemia in heart failure (HF), the Reduction of Events by Darbepoetin Alfa in Heart Failure (RED-HF) study, a large phase III trial, demonstrated that darbepoetin alfa was not able to improve clinical outcomes in patients with HF who had reduced ejection fraction and mild to moderate anemia, and indeed, it led to an excess of thromboembolic events.[1] In view of those results, recent guidelines do not recommend ESAs for the treatment of anemia in HF patients.[2]

Anemia in Heart Failure Patients

Heart failure is a very common disease, with severe morbidity and mortality, and a frequent reason of hospitalization. Anemia and a concurrent renal impairment are two major risk factors contributing to the severity of the outcome and consist of the cardio renal anemia syndrome. Anemia in heart failure is complex and multifactorial. Hemodilution, absolute or functional iron deficiency, activati...

Anemia and iron deficiency in heart failure.

Anemia is a common comorbidity in patients with heart failure (HF) and is associated with poor prognosis. Iron deficiency, with or without anemia, confers increased risk of mortality and morbidity. Along with the altered iron metabolism in HF patients, inflammation creates challenges in the interpretation of laboratory parameters used to diagnose anemia in HF. Since the RED-HF trial failed to d...

Effects of erythropoiesis-stimulating agents on heart failure patients with anemia: a meta-analysis

INTRODUCTION Heart failure (HF) is always complicated with anemia and is associated with bad prognosis in this patient population. Several studies have assessed the potential role of erythropoietin-stimulating agent (ESA) in improving cardiac function and reducing the number of hospitalizations in anemic patients with HF. AIM We performed a meta-analysis to assess the potential role of ESA in...

The role of erythropoietin stimulating agents in anemic patients with heart failure: solved and unresolved questions

Anemia is a common finding in congestive heart failure (CHF) and is associated with an increased mortality and morbidity. Several conditions can cause depression of erythroid progenitor cells: reduction of iron absorption and reuptake, decreased bone marrow activity, reduced endogenous erythropoietin production, and chronic inflammatory state. Anemia's etiology in CHF is complex and partially u...

Should erythropoiesis-stimulating agents be used in predialysis patients?

Anemia results from insufficient production of erythropoietin by the kidneys and is a common complication of chronic kidney disease. It is present in most patients with stage 4 and 5 chronic kidney disease. Studies have linked untreated anemia in chronic kidney disease to worsening cardiovascular complications, possible acceleration to end-stage renal disease, and death. Administration of eryth...
